Access other accounts

With MyLGHealth, you can access and manage the healthcare of your children, spouse or parents. Through proxy access, you may access your child's records until they reach age 13.

From the age of 13 to 17, parents or guardians will not have proxy access to their child's medical information through MyLGHealth. As always, parents or guardians may contact their doctor's office to discuss their child's healthcare.

You must create your own MyLGHealth account in order to be proxy to another individual. Once you sign up, you can view your proxy's MyLGHealth account under the "My Family's Records" section.

Request a MyLGHealth account or Minor Proxy access

You can now request proxy access for minors (Patients age 12 and under) online from your MyLGHealth account. Login and go to My Family's Records, Child Proxy Request.

If you prefer not to sign-up online or at your doctor's office, please print the form below and an activation code will be mailed to your home.

  1. Print and complete form:

    Minor Consent to Signup Form - Patients 13 to 17 years of age.
    Minor Proxy Form - Patients age 12 and under.
    Consent to Signup Form - Patients 18 years of age and older.

  2. Completed forms may be sent by the following methods:

  3. You will receive an enrollment letter in the mail with an activation code. Once you have the activation code, click here to set-up your MyLGHealth account.

Adult to Adult Proxy

For adult proxy both adults must provide consent for access to occur, unless a legal guardianship has been established. Adult proxy forms must be signed and witnessed by an LG Health employee at an LG Health facility such as your primary care provider, specialist or one of our lab facilities:

Adult Proxy Form - Patients 18 years of age and older. (Patient granting proxy access must sign in front of LG Health employee witness).
